Strive Healthcare is a rehabilitation company offering Physical therapy, Occupational therapy, and Speech therapy in the Home Health and Outpatient settings. Strive Healthcare is located at 165 Silver Lane, St. Augustine, FL inside the beautiful Silver Creek Assisted Living Facility and at 203 S. Moody Rd. Palatka, FL at Vintage Care Senior Living. Strive operates a state of the art outpatient facilities offering sports medicine, vestibular rehab, manual therapy, orthopedic rehab, geriatric care, memory care, ADL training, balance training, swallow analysis and training, strengthening, and injury prevention through physical, occupational, and speech therapy services. Strive Healthcare serves both the residents of Silver Creek and is open to the public for traditional outpatient therapy services.

We then utilize the latest technology advancement in therapy and our vast experience to achieve your goals.
Home Health Care Services are staffed by Strive Healthcare through our partnership with Trilogy Home Healthcare and Interim Healthcare. Strive Healthcare offers Nursing, Physical Therapy, Occupational Therapy, and Speech Therapy.
Outpatient Services offered are Physical therapy, Occupational therapy, Speech therapy.
Dr. Laura Hemelt, a Doctor of Physical Therapy, is the owner of Strive Healthcare. Laura graduated from Florida State University in 1999 with a degree in exercise science and continued her education at the University of St. Augustine, where she completed her Doctorate of Physical Therapy degree in 2003. Laura’s career has consisted of being a director of rehabilitation at an outpatient therapy office and directing physical therapy services at Assisted Living Facilities. In addition to her professional career, she is a mother of 5 children. She is married to Dr. Ryan Hemelt, who also practices physical therapy and occupational therapy. They have a combined 30 years of experience in the fields of physical and occupational therapy. Laura is a first generation Cuban American and is bilingual with a fluency in Spanish.
Dr. Ryan Hemelt is dually credentialed as both a Doctor of Physical Therapy and an Occupational Therapist. Ryan has been in practice since 2003 and serves as the current director of Rehabilitation for the largest orthopedic group in the St. Augustine area as well as supporting his wife as the owner of Strive Healthcare. When Ryan is not serving patients at Orthopaedic Associates or caring for the residents at Silver Creek, you may find him at the YMCA where he is on the Board of Directors and coaches youth sports. In addition, he also serves on the board of the Ancient City Soccer Club and coaches youth soccer as well as serves on the board of the Pediatric Orthopedic Fund through Flagler Hospital. Ryan and his Family are members of St. Anastasia Catholic church and have 5 children, 3 of which attend the Cathedral Parish School. Ryan and his family are members of Marsh Creek Country Club, love sports and the outdoors. Ryan’s principal interests in his practice are in sports medicine and orthopedic rehabilitation.
Kyshia Taylor, RN BSN has been working in home health care for nearly five years. Kyshia grew up in the Gainesville area and went to school at the University of Florida where she received a bachelor’s degree in psychology. After college she managed two physician practices, an internal medicine practice as well as a general surgery practice. While working at those practices and while implementing several patience assistance programs, she decided she wanted to provide direct patient care. Therefore, she went back and got her bachelor’s degree in nursing from the University of North Florida. Kyshia is passionate about her patients and the care she delivers. She enjoys the one on one care and education she is able to provide to her patients. In her free time, Kyshia enjoys spending time with her husband and 3 daughters. She also enjoys dancing, going to the beach, and spending time with friends.
I work with passion. Passion for people, my craft, and my community. I feel that my professional and personal experience has equipped me to blend all three into a successful career. I started out in the profession of Physical Therapy in 2002 and enjoyed living across the country serving 13 week contracts in various clinical settings. My wife and I met great people, learned tons about the business, and learned our respective specialized settings. She chose Pediatrics and I chose Vestibular therapy, primarily in the home health setting. Over the course of the past 14 years, I have been lucky enough to be surrounded by strong clinicians, leaders, and mentors, that taught me to look at the forest, not the trees. I believe that my most current leadership roles has actually helped my clinical delivery to patients. Treating patients with a holistic approach and leaning on my peers in nursing, social work, and other therapies has yielded strong patient outcomes and a growing clinical reputation. I am currently co-owner of Next Step Physical Therapy, a pediatric horse based therapy practice, and co-founder of Horse Play Therapy, a non-profit organization, that meets the financial demands of children to receive horse based therapy. My wife and I still save time for our greatest treasures, our two boys. Between soccer, camping trips, and all the rigors of parenting, I stay busy.
Jessica Harris, MS, CCC-SLP is a graduate of Appalachian State and Florida State University. The NC native developed a deep and personal interest in speech therapy following her father’s stroke. Jessica has clinical experience in acute care, pediatrics, and several specialties in adult care including LSVT for voice, Myfacial release, and NMES for swallowing. She lover running, biking, and spending time outdoors with her husband and 2 daughters.
Arlyn Rivera Jackson was born and raised in Puerto Rico and moved to Jacksonville, Florida in June 1991. She is married to Greg Jackson and they have two sons, Bryce and Kyle who are the pride and joy of their lives. Arlyn graduated with a B.S. in Exercise Science from Florida State University in May 1999 and went to earn a Master’s in Physical Therapy (MSPT) from the University of St. Augustine in January 2003. She has specialized in pediatrics since 2003 and has worked in various settings such as home health, outpatient and acute care in St. Augustine and Jacksonville, Florida. In her personal time, Arlyn enjoys running with her oldest son, taking her youngest son surfing, and camping with her family.
